docs: fix stale Known Issues list in CLAUDE.md
Four of the six tracked issues (#228, #234, #256, #260) had shipped since this list was last touched, and #250 was closed too — the instructions Claude reads at session start were actively wrong. Co-Authored-By: Claude Sonnet 5 <noreply@anthropic.com>
